SMS / Text Messaging Terms & Conditions

These SMS / Text Messaging Terms & Conditions (“Terms”) govern your consent to receive text messages from Desert Sky Family Dental, LLC, d/b/a Sky Dental, on behalf of its affiliated practice locations including Alta Sky Dental, Desert Sky Dental, Sierra Sky Dental, Southern Sky Dental, and Chandler Sky Dental (collectively, “Sky Dental,” “we,” “us,” or “our”). By providing your mobile phone number and checking the consent box on our appointment form, website, or other intake forms, you agree to these Terms.
1. Description of the Messaging Program
By opting in, you consent to receive recurring text messages from Sky Dental at the mobile number provided, including:
- Appointment reminders, confirmations, and rescheduling notices
- Promotional offers, discounts, and referral program updates (e.g., new patient specials, orthodontic offers)
- Account, billing, and payment-related notices
Message frequency varies depending on your appointments, account activity, and ongoing promotions. You may receive more than one message per week.
2. Consent to Receive Messages
By checking the SMS consent checkbox on our website intake form, you expressly consent to receive text messages from Sky Dental related to the categories described in Section 1. Consent to receive text messages is not a condition of purchasing any goods or services. Submission of your phone number through any other means (verbally, by phone, or on a paper form) does not, by itself, constitute consent to receive text messages — written opt-in through the designated checkbox is required.
3. Eligibility
You must be 18 years of age or older to use this SMS service. By opting in, you represent that you are at least 18 years old and the account holder for the mobile number provided, or that you have the account holder’s permission to provide consent on their behalf.
4. Message and Data Rates
Message and data rates may apply, depending on your mobile carrier and plan. Sky Dental does not charge for text messages sent as part of this program, but your carrier’s standard messaging rates may apply to the messages we send and that you send to us.
5. How to Opt Out
You may opt out of receiving text messages at any time by replying STOP to any message you receive from us. After you send STOP, we will send a one-time confirmation message that you have been unsubscribed, and you will no longer receive text messages from this program. If you would like to resume receiving messages, reply START or re-opt in through our website intake form.
Opting out of text messages will not affect your ability to schedule or receive dental care at Sky Dental. We will continue to contact you by phone or other means as needed for appointment-related communications.

7. Supported Carriers
This program is supported on major U.S. wireless carriers. Carriers are not liable for delayed or undelivered messages. T-Mobile is not liable for delayed or undelivered messages.
8. Privacy
We respect your privacy. Your mobile phone number and any information collected in connection with this text messaging program will be used solely to provide the services described in these Terms and will not be shared with third parties for marketing purposes. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third party.

9. Changes to These Terms
Sky Dental reserves the right to modify or discontinue this text messaging program, or these Terms, at any time. Updates will be posted on this page with a revised “Last Updated” date. Continued participation in the program after changes are posted constitutes acceptance of the updated Terms.
